I understand and acknowledge that participating in pilates and yoga classes involves inherent risks and hazards including but not limited to to physical exertion, potential for injury or death, and the risk of contracting illnesses. I voluntarily assume all such risks and responsibilities associated with my participation. I confirm that I am in good physical condition and that I have no medical conditions that would prevent me from safely participating in said classes or related activities. I understand that it is my responsibility to consult with a healthcare professional before participating in any physical activities and that PRANA/LIFE LLC instructors are not medical providers and do provide medical advice. I release, waive, discharge, and covenant not to sue PRANA/LIFE LLC, its owners, instructors, employees, and volunteers (collectively referred to as "the Releasees") from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of any damages, losses, or injuries, whether physical or emotional, including but not limited to those caused by the negligence of the Releasees, arising out of my participation in any classes and services on site. I further acknowledge that PRANA/LIFE LLC is in no way responsible for the safekeeping of my personal belongings. I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost they may incur as a result of any claim made against them by me or on my behalf. If any provision of this Waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect. This Waiver shall be governed by and construed in accordance with the laws of the state of Michigan, and any legal action or proceeding relating to this Waiver shall be brought exclusively in the state or federal courts located within the state of Michigan. By signing below, I acknowledge that I have read and understood this Liability Waiver in its entirety and agree to be bound by its terms and conditions. If the participant is a minor, the parent or legal guardian's signature is required.
